The Newfoundland and Labrador Centre for Health Information (NLCHI) maintains this COVID-19 Assessment and Reporting web application on behalf of Public Health and the Government of Newfoundland and Labrador for the purpose of helping you assess your symptoms and, if required, schedule your COVID-19 test. In addition, the information you provide will be used to assess whether you are eligible for COVID-19 medications and treatments (i.e., Paxlovid).NLCHI is committed to protecting the privacy of individuals who visit this website in compliance with the Access to Information and Protection of Privacy (ATIPP) Act and the Personal Health Information Act (PHIA).
Personal Information
Personal information means recorded information about you as an identifiable individual but is not limited to contact information (email address, phone number, name, age, and gender). Personal health information means oral or recorded identifiable information about an individual that is collected, used, disclosed or processed for the provision of healthcare. This includes name, date of birth, MCP number, COVID-19 symptoms, and rapid self-test results, etc.
To the extent that information is collected or provided through the website and can be used to identify an individual, we will treat it as confidential in accordance with this Privacy Notice.
Collection, Use & Disclosure of Information
When using the website, you will be asked to provide certain Personal Information or Personal Health Information including name, date of birth, MCP number, COVID-19 symptoms, and rapid self-test results, etc. We will submit your information to Public Health for scheduling and to health care providers if it is determined that you are a potential candidate for COVID-19 medications and treatments (i.e., Paxlovid).
This information will be retained as part of your patient health record and collected, used and disclosed for your health care as well as for public health surveillance as permitted by s. 34 (m) of the Personal Health Information Act. Other uses and disclosures as required by law may also occur. Where possible, identifying information will be removed prior to any use or disclosure.
Compliance
Questions, concerns, or complaints relating to the COVID-19 Assessment and Reporting web application’s privacy policy on the treatment of your personal information should be directed to privacy@nlchi.nl.ca.
If your complaint is not resolved to your satisfaction, the ATIPP Act and the PHIA provide a mechanism for independent review by contacting the office of the Information and Privacy Commissioner – https://www.oipc.nl.ca/
